Our Faith Formation program offers religious education for the following:

To request a copy of a Baptism, First Communion or Confirmation certificate, please contact the parish office at 630-837-5411 or by email at info@stpeterdamian.org. There is a $10 processing fee for this request. We accept cash, credit card using our e-giving website, or check made payable to St. Peter Damian. Please be ready to provide the following information:

• Full name on the requested certificate

• Birth date

• Where the certificate will need to be sent 

• Method of payment (cash, check, or credit card)
