BAPTISM
If this is your first time bringing a child forward for baptism, or you have not attended a baptismal preparation class within 5 years since your last child was baptized, parents are required to attend a baptismal preparation class prior to scheduling a date of baptism. To attend this baptismal preparation class, please contact the parish office at
630-837-5411 or email info@stpeterdamian.org. Baptismal preparation classes are held once a month on a Tuesday evening. If you have completed a baptismal preparation class in the last 5 years, please read through ALL the information provided and complete the BAPTISMAL INTAKE FORM at the bottom.
Please note: If a completed baptismal intake form is submitted this does not mean your baptism is scheduled. A review of required documents will be made you will be contacted.
